Combatsims Waiver

Please fill out and sign this waiver and turn in to the event coordinator.  The waiver is good for 1 year.  This waiver must be turned in beofre you participate in any Combatsims organized event, regardless of field location.  If you are under 18 years old your parent/guardian must sign this form.

CS Waiver

Hawaii All Star Paintball (on Nimitz Hwy)

Kualoa Waiver

Please note that those 18 years or older are able to sign in @ Kualoa Ranch's E Komo Mai building and do not need this waiver.  Those who are under 18 years of age must have a parent/guardian sign this waiver and bring it with them.  For further questions you can contact Kualoa Ranch :(808) 237-8515

Rental Agreement

Please note that those 18 years or older MUST sign the rental agreement and are fully responsible for any damages or lost items.  Those who are under 18 years of age must have a parent/guardian sign this rental agreement and bring it with them.  It is a $30 deposit for each set.  Please contact Animaniac via the forum with any questions or reservations for equipment rentals.  You need to notify him at least 48 hours from the game date to reserve your rental.  This will give him the time necessary to prepare the equipment for rental.
